All others, please download and print the form, then mail it in with your cheque.

DIRECTIONS:

1.	Please fill out the online application form completely and accurately including your credit card information.
2.	Once completed, click File, then Save As to save the file as a PDF document on your computer.
3.	Open your email program and attach the PDF document, then send to: cherith.ont@sympatico.ca.
4.	Please click next to complete the online application.
